I was ‘lucky’ enough to get assigned to a project that unfortunately suffered from a bad design that was furthermore poorly implemented.  Additionally, there was no CM controls, so I was not shocked when the inevitable delivery mis-queues and production issues arose. 

There was a meeting where the customer asked, “What changed in the release?”

Our ‘Senior’ developer didn’t hesitate to exclaim that “We don’t know!” which mortified my Project Manager (who was also somehow ‘lucky’ to be assigned to that debacle of a project).
